When accidents (or near-misses) happen it is important to learn from them.

Key information / learning points include:

• Identifying the mistakes that led to the incident
• Identifying the necessary changes to put in place to rectify / avoid them so that they don’t happen again
• In some accidents, it is also important to know who is to blame in order to protect you, and your business.

The investigation process generally includes:

• interviews
  • conducted with appropriate persons
• inspections
  • of the area(s) where the accident took place
•  forming an opinion of
  •  the sequence of events
  • contributory factors leading to the accident
• recommendations
  • where applicable, recommendations will also be given on how to avoid similar accidents in future.
